Due to the COVID-19 pandemic, an electronic version of your material is attached so that your distribution may be more simplified. The electronic version may be distributed through mass email along with posting on your website in an area where retirement or benefit information can be found.

If you would like to receive printed copies of the 2023 Meaningful Notice instead, please notify us by Monday, February 1, 2023.

PPO Plan

The PPO Plan (http://www.bcbsil.com) is a self-insured plan administered by Blue Cross. Please see the Summary of Benefits and Coverage for more details.

$750 Deductible PPO Plan

The $750 Deductible PPO Plan is only available to staff under the Teachers' contract. Please see the Summary of Benefits and Coverage for more details.

HMO Illinois

The district also offers HMO Illinois (http://www.bcbsil.com) as an alternate to the PPO. Please refer to the Summary of Benefits and Coverage for more details.

Blue Advantage HMO

The district also offers Blue Advantage HMO (http://www.bcbsil.com) as a second HMO option.

High Deductible PPO w/HSA

The district now offers a High Deductible Health Plan as another alternative to the traditional PPO. Please see the Summary of Benefits and Coverage for more details.

Blue Access for Members

  • Get information about your health benefits, anytime, anywhere. Use your computer, phone, or tablet to access the BCBSIL secure member website, Blue Access for Members (BAM): https://www.bcbsil.com/member
  • With BAM you can securely access your account to review your coverage and claims status, find wellness resources, request a new ID card, search for doctors and more
  • With BAM you can use Provider Finder to choose in-network doctors and hospitals, compare cost and quality, and estimate out-of-pocket cost before making treatment decisions.

Fitness Program Membership

  • Members with PPO or HMO medical coverage may enroll in the BCBSIL Fitness Program. The program gives you unlimited access to a nationwide network of more than 9,500 fitness locations.
  • Monthly fees are $25 per month per member with a one-time enrollment fee of $25 per member. Fitness Program Membership Flyer

Virtual Visits -Convenient health care at your fingertips

  • PPO members have access to the services of MDLive for on-demand care for non-emergency medical and behavioral health services. Members may connect with a board-certified physician either by telephone, computer or smart phone.

Vision Coverage:

  • With HMO Medical Coverage you have vision coverage with Eyemed. To print or download a vision card please register online: https://www.eyemedvisioncare.com/bcbsil/public/login.emvc.

  • When searching for a provider through the website the employee can type their zip code and narrow down the search by importance level (hours & scheduling, services, products, and select brands)

  • With EyeMed each dependent is allowed one eye exam every 12 months and discounts on glasses and/or contacts.

  • With PPO Medical Coverage each dependent is eligible for a $200 stipend every 24 months. To submit for reimbursement please complete and return the BlueCross BlueShield of Illinois Claim Form to: Ms. Rosie Berry (email: [email protected], mail, or interoffice).

Dental Coverage:

Waukegan School District offers one dental PPO Plan. It is a self-insured plan administered by Blue Cross/Blue Shield. For details, please visit the Dental Benefit Highlights sheet.

Cafeteria Plan (Section 125 Flexible Spending Program):

An added benefit is the district's Flexible Spending Program. An employee may take advantage of the pre-tax benefits by using it for:

  • Dependent Care
  • Unreimbursed medical and dental expenses
  • Pre-taxing dependent premiums

The Cafeteria Plan's open enrollment is in November, and the benefit period is January 1 - December 31.

PayFlex Flexible Spending Account (FSA)

Employee Assistance Program:

The District offers an EAP program through National Insurance Services. The EAP covers a wide range of issues that an employee may be experiencing.
