Reminder of Health Requirements for Students

The Illinois Department of Public Health requires a number of immunizations and exams before students can attend school in the State of Illinois. Below is a detailed list of those requirements, and the grades affected by each prerequisite.

All students transferring from an out-of-state school, as well as any student entering early childhood, kindergarten, first grade (if student didn’t attend Kindergarten), sixth grade or ninth grade are required to have a completed exam, along with all required immunizations. Any students transferring into the District from another school in Illinois are required to submit a transfer form that details his or her exam and immunization record.

The Illinois Department of Public Health requires that District 60 maintain health records for every student. Those records must include a complete record of immunization, indicating dates that the student received diphtheria, tetanus, pertussis, polio, measles, mumps, rubella, and Hib & Hepatitis B (preschool only). Every student, up to and including tenth grade, must show proof of receiving the varicella vaccine or show proof of disease history. Every student who enters the fifth grade must show proof of hepatitis B immunization. These records should be submitted prior to the start of school unless presented in a previous school year.

Additionally, a vision exam is required for all students entering Kindergarten, or new students entering Illinois for the first time from out of state or out of country. The exam is due by October 15, 2012, and must be completed by a licensed eye care professional. A dental exam is required for Kindergarten, second, and sixth grades and is due by May 15, 2013.
