The final new student registration event for Waukegan Public Schools will take place on Thursday, August 4, between 1 p.m. and 7 p.m. at the Welcome Center, 742 Greenwood Avenue.
Any families new to the District who have not yet registered for the 2011/2012 academic year are encouraged to do so on August 4. If parents/guardians cannot attend the final new student registration, they will need to register at the Welcome Center beginning on August 8 between the hours of 8 a.m. and 3 p.m.
Parents are asked to please bring their child’s original birth certificate, as well as a proof of residency. Proof of residency must include two of the following: a gas or electric bill dated within the past two months, lease or housing contract showing parent’s name and address, or if a parent or guardian lives with someone else, a notarized affidavit letter from that person with a gas or electric bill in that person’s name will be required. No child will be registered without all of the required documentation.
Parents are also asked to bring a copy of their child’s immunization, health, dental, and vision records. If transferring from another public school in Illinois, parents will also need to bring an Illinois Student Transfer Form from their child’s previous school.
